·We have been providing wellness services for more than 20 years for corporations ranging from 25 to 10,000 employees in cities throughout the state and U.S.
·Based on the outcomes of a cross section of our corporate clients, on average, we reduce or eliminate 46% of employee risk factors over one year with services including lifestyle coaching and pre and post health screenings/assessments.
Health screenings are provided at the work-site. Blood tests use a finger stick with immediate results. Individuals receive 1:1 confidential feedback about their test results from a health promotion specialist.
Health Risk Assessment
·Health Risk Assessment (HRA): A self-reporting questionnaire that assesses a participant’s physical and mental health risks, readiness to change, and health topics of interest. The assessment includes questions about lifestyle behavior, family history, and factors associated with incidence of serious disease. Each participant receives a comprehensive, confidential individual report. The HRA and reports can be paper or online.
·Individual Report: Participants receive a detailed report (on-line or paper), including their bio-measures from the health screening, physical and mental risk factors, and recommendations for improvement. Individual results are confidential and data reporting complies with HIPAA privacy standards.
·Group Summary and Executive Summary Reports: Employers receive aggregated reports of employee physical and mental health risk factors, readiness to change, and health topics of interest to their employees. Health Promotion staff provide consultation and recommendations for future programming based on HRA and screening results. Note: If there are less than 25 participants, these reports will not be provided due to concerns about confidentiality.
Health Screening Services
·Total Lipid Profile with Glucose: This test is the most comprehensive screening using finger-stick technology and provides readings of total lipids (total cholesterol, HDL, LDL, triglycerides), total cholesterol/HDL ratio, and glucose. Test results can highlight risk for heart disease, stroke, and diabetes. There is a recommended fasting period of 8-12 hours for this test to be most accurate.
·Blood Pressure Test:This is a reading of the pressure the blood is putting on the blood vessels. High blood pressure can damage vessels leading the heart, brain, eyes, and kidneys. Risk for heart attack or stroke doubles for every 10-point increase in systolic and 20-point increase in diastolic rates. It is important to check blood pressure regularly, because there are no warning signs for high blood pressure.
·Body Mass Index: Body Mass Index (BMI) is calculated from a person's weight and height. BMI provides a reliable indicator of body fatness for most people and is used to screen for weight categories that may lead to health problems such high blood pressure, heart disease, stroke, diabetes, and other illnesses.
·Body Fat Analysis: Body composition is measured using a noninvasive analyzer. The analyzer determines the percentage of fat tissue through a brief body scan, which takes approximately 2-3 minutes per participant. High body fat levels can lead to high blood pressure, heart disease, stroke, diabetes, and other illnesses.
·Waist Circumference: This can be used alone as an indicator of health risk due to abdominal obesity being the issue. Individuals with more abdominal fat are at increased risk for high blood pressure, diabetes, high cholesterol, coronary artery disease and premature death.
·Flexibility Screening:This tests hamstring and lower back flexibility using a sit-and-reach exercise. Flexibility is important for preventing back strain and other injuries.
·Prostate Test: The PSA (Prostate Specific Antigen Test) is performed at the work-site with a finger-stick. This test measures the amount of a particular protein that can be found in the blood. If the PSA level is too high, further testing is suggested to determine if the cause is prostate cancer. Men with a family history of prostate cancer as well as African-American men should be tested at age 45. All other men should receive this test beginning at age 50.
·Fitness Assessment: This measures body weight, blood pressure, heart rate, body fat, circumference, cardio-respiratory fitness, flexibility, muscular strength and endurance. The participant will receive an individualized computer report with results and comparisons to the general population (based on age and gender). Recommended for ages 15-60.
·Nutrition Assessment and Counseling: A registered dietician provides a confidential, 1:1 consultation with each participant to assess eating and nutritional habits, identify areas for improvement, and develop a plan for healthier eating. The dietician teaches participants ways to make healthier food choices, taking into account each individual's situation, goals, and needs.
·Seminars: The Community Health Network offers more than 100 experiential and educational seminars on a variety of topics, including health and wellness, chronic health conditions, work-life balance, relationship and management skills, among others. All seminars are provided by experts in the field, and can be customized to meet the needs of an organization.
·HeartSaver CPR, First Aid Training, Bloodborne Pathogens: Courses provide 2 year certification for CPR and AED for adults, children, and pediatrics, and first aid skills. Participants learn to assess and maintain life following an emergency until the arrival of emergency medical services personnel. Courses follow American Heart Association regulations for CPR, AED, and first aid, and OSHA standards for bloodborne pathogens.
·Wellness Newsletter:An easy to read, informative newsletter that focuses on a variety of health and wellness topics. The newsletter can be sent monthly or quarterly to the employer or employee homes via mail or e-mail.
·Health Handbook:This user-friendly book makes it easier for employees and their families to take responsibility for their own health and well-being. The book provides information on how to be a wise medical consumer, guidelines for using the health care system, wellness and preventing disease, and treatment decision trees for more than 200 ailments. We can also provide on-site education and orientation on how to use the books. Available in English and Spanish, and/or with corporate logo.
·E-health Multimedia Library:We can customize a web portal foryour employees to access Community Health Network's extensive on-line health library. Employees can access health management tools and educational materials, including health risk assessments, calculators to measure risk factors such as body mass index, and information ranging from the common cold to heart disease. In addition, employers can post internal human resource information such as benefits booklets, wellness program calendar, etc.
·Personal Wellness Coaching:25% of employees screened with the most risk factors, will be identified from the health screening/HRA and assigned to a Personal Wellness Coach for confidential, telephonic coaching and intervention. Risk factors include: nutrition, exercise, stress, weight, blood pressure, cholesterol, glucose, tobacco, alcohol, etc.
Coaches are experts in nursing and/or health-related fields and utilize Prochaska’s Stages of Change and Intrinsic Coaching models to determine an individualized coaching approach with each participant. Coaches contact participants regularly throughout the year to help them design a plan and set realistic goals to reduce and/or eliminate risk factors. Coaches provide education, resources, motivation, and guidance for making sustainable, healthy lifestyle changes.
·Freedom from Smoking: This is an American Lung Association Program that promotes various smoke free strategies, including understanding recovery symptoms, nicotine replacement, and drug tracking. This program is offered at the work-site, once per week for one hour for a total of 8 weeks.
·Smoking Cessation Programs: programs include educational materials, and can include onsite 8-week class, individual telephonic coaching, and nicotine products (gum or patch).
·Maintain No Gain Holiday Weight Program:Program runs Thanksgiving to New Year’s and helps participants maintain weight (and gain no more than 2 pounds) during the holiday season. The average person gains 7 pounds during the holiday season and then finds it hard to shed extra pounds. Program includes an initial weigh in to establish a base weight, and a final weigh out at the close of the program. Participants receive log books, educational materials, and a healthy recipe book.
·Exercise America, Peak Fitness, or Hawaiian Challenge:This 6 week program provides all the tools needed to plan and track daily exercise habits “virtually traveling” across America, Hawaii or climbing mountains. Participants receive weekly hand-outs and a logbook to track their daily exercise minutes. Participants receive a chart to convert exercise minutes into miles traveled across America or mountains climbed. The chart is different for beginner and advanced exercisers.
·Walk Your Way to Wellness 10,000 Steps a Day:This program encourages participants to gain a better understanding of the distance they walk at the onset of the program, and increase their level of activity to 10,000 Steps a Day. Inactive people typically take 2,000-4,000 steps per day. By increasing daily activity to 10,000 steps, participants are walking an average of 3-5 miles. All participants receive a pedometer, handouts, and a logbook.
·Health Promotion Services plans, designs, promotes, develops and manages a wide range of incentive program for our clients. Incentive programs include cash, prizes, and health care premium discounts for participation in wellness activities and/or healthy behaviors or outcomes (i.e. healthy blood pressure, weight, smoke-free, etc).
·Health Promotion Services tracks outcomes and can provide your human resources or accounting department with financial incentive amounts, while maintaining HIPAA confidentiality regarding individual employee health information.
Organizational Effectiveness programs focus on the overall health of the organization. The purpose is to create an organizational environment where individuals and teams thrive. Services include assessment, education, and training in areas such as leadership development, communication, performance management, change management, etc.
·Assessment: Assessment can include focus groups, interviews, and questionnaires to assess the health of an organization and provide strategic recommendations for next steps. The standard health risk assessment (HRA) includes questions related to an individual’s physical and mental health. The HRA can be customized to include questions related to employee perception of the overall effectiveness, health, and culture of the organization, such as leadership, vision, teamwork, work environment, etc. Varies
·Picture This Interactive Business Theatre: uses improvisational theatre and role-playing to involve groups in problem-solving challenging work and life issues. Picture This actors create a dynamic, entertaining, and open forum for the exchange of diverse thoughts, feelings, and values. Picture This topics can be customized to meet the needs of any organization. Popular topics include communication, diversity, sexual harassment, performance management, executive coaching, and change management $675 per hour
·Relationship Development: This experiential seminar focuses on communication, listening, and relationship development skills. The seminar promotes increased self-awareness, accountability, and effective responses to change, conflict, and problem solving. Participants learn to work together in a positive atmosphere of team building, honesty, and collaboration. The training session is typically a 2-day experience, with additional follow-up. However, the session can be offered in 2-3 hour segments. $250 per hour
·Myers-Briggs Test & Training: The MBTI distinguishes different personality profiles and ways that people gather information, make decisions, and interact with other people. The MBTI is a highly effective tool for team building, organizational development, business management, career counseling, and leadership training. Certified MBTI facilitators administer and interpret MBTI for individuals and/or teams. $250 per hour +$30/person material fee
·Employee Assistance Program (EAP): Licensed counselors provide short-term therapy to address diverse home and work issues. Employees or anyone living in their household can receive up to 8 sessions per issue per person. Employees can refer themselves, or be referred by a supervisor. EAP services also include 24-hour phone access, crisis intervention, critical incident stress management (CISM) for employees exposed to a crisis event, management consultations, promotional/educational materials, and annual orientations. EAP sites are established within 30 minutes of an employee’s home or work, anywhere in the U.S.
·EAP Outreach Program: This program is additional to the traditional EAP program, as EAP therapists initiate phone calls with participants with mental health risks identified by the health risk assessment. We recommend following up on employees who drink more than 1-2 drinks per day and/or have 2 or more of the following: difficulty with coping status, feelings of down heartedness/blue, feelings of worthlessness/inadequacy/unimportance, bodily pain, anxiety or depression. EAP therapists will make up to 3 outreach calls to the at-risk employee and attempt to connect the employee with EAP and/or other resources needed for recovery. On average, approximately 3% of participants will qualify for this outreach program. You will receive a report with the number of outreach calls and general outcomes.
